Not that big a deal really, I suspect it would be something similar to economizers that are used on boilers.  It's a fin tube heat exchanger which is placed over the exhaust stack outlet.  Water runs through the fin tube coil and is heated.  It's not like the water would come in contact with vaporized bodies.  It's amazing how much energy we waste every day via spent exhaust gasses.

I had a water heater like that in Ireland. We burned peat in the fireplace and the flue had a coil to collect the heat.  It circulated water into a copper tank in a big cabinet right next to the fireplace.  We insulated the tank with folded towels so that when anyone wanted to take a shower, there were hot towels available.  It didn't hold much, but it was a nice set up.  The peat burned down to fine ashes like charcoal does.  I could live like that, but SWMBO and some teenagers could not.
